New Patient Registration
Patients wishing to register must attend the surgery to complete the appropriate form and to provide two forms of proof of address confirming they reside within the Practice area.
In the case of a new born baby it is only necessary for the parent to attend and complete the appropriate form.
Once registered, should you move to a location that is outside the practice boundary (see below) you will be required to register with a new doctor. You will not be permitted to remain on our list of patients.
It is the responsibility of the patient or the guardian of the patient to inform the surgery should any changes take place that could affect their medical care. The main issue here is with address and telephone number changes, also change from maiden to married names.
Patients are registered with the practice and not individual doctors. It is completely fine to request an appointment with a particular doctor for continuity of care, etc. in the majority of cases we will be able to provide this, however there are times when it cannot be guaranteed.
Some years ago the practice boundaries for new patients were changed, see practice area map, this meant we no longer accept new patients from Shifnal, Codsall and Perton, for example. We felt, when this decision was made that it was unacceptable for existing patients in these areas to be asked to register with new practices and so they were kept on our list. However when such a patient changes address unless the new address is within our current practice boundary they will be asked to register with a new practice.
